AI Hospital Readmission Risk Stratifier

Paisley Suzuki

Full-time · Job · Chicago

About the role

CMS penalizes hospitals financially for excess 30-day readmissions. A patient readmitted within 30 days of discharge for congestive heart failure costs the hospital both the readmission care cost and the CMS penalty applied to all heart failure DRG payments for the year. Hospitals that can accurately stratify discharge patients by readmission risk and intervene for high-risk patients reduce readmissions, improve patient outcomes, and avoid millions in penalties. We serve 32 health systems with predictive analytics capabilities. Readmission risk stratification is one of our most operationally impactful products: the model output goes directly into the discharge planning workflow, where care coordinators use it to decide which patients need post-discharge follow-up calls, home health referrals, or pharmacist medication reconciliation. You'll maintain and improve the readmission risk model: retraining on updated claims and clinical data, evaluating performance across patient populations and hospital units, implementing bias monitoring ensuring the model doesn't underpredict risk for specific demographic groups, and building the model output integration that surfaces risk scores in the care coordination workflow.
