About the role

We serve over 40 hospital networks that need better visibility into patient outcomes after discharge. Readmission penalties and quality metrics keep rising, and clinical teams cannot manually track every patient once they leave the building. You will design and productionize models that flag patients at high risk of post-discharge complications using structured EHR data, lab trends, and medication history. This means close collaboration with clinical informaticists and a strong bar for interpretability, since every score needs to be defensible to a physician. The models you ship will directly influence discharge planning and follow-up scheduling for thousands of patients a month, reducing avoidable readmissions and giving care coordinators an earlier warning window.

Responsibilities

  • Build risk-scoring models on structured EHR and lab data
  • Validate model outputs against clinical outcomes with the informatics team
  • Maintain model monitoring dashboards for drift and calibration
  • Document model assumptions for hospital compliance review
  • Partner with care coordinators to refine risk thresholds

Requirements

  • 5+ years building ML models on healthcare or clinical data
  • Comfortable explaining model behavior to non-technical clinicians
  • Strong Python and SQL skills
  • Experience with model monitoring in production
